Learning Objectives

  1. Identify common SDOH screening tools and outline strategies for their effective use in various oncology practice settings.
  2. Describe the roles of the multidisciplinary oncology team in systematically addressing SDOH needs across the cancer care continuum.
  3. Explore evidence based interventions such as patient navigation, financial counseling, transportation assistance, and telemedicine to address identified social needs in cancer care.
  4. Distinguish implementation barriers and facilitators across diverse oncology practice settings including staffing, reimbursement, and data infrastructure
  5. Discuss sustainability strategies for SDOH focused programs, including funding models, community partnerships, and integration into value based care frameworks.
