EMPOWERING THE MEDICALLY INTEGRATED TEAM
This white paper highlights the implementation of NCODA’s Proactive Symptom Management in Myelofibrosis Positive Quality Intervention (PQI), which empowers medically integrated teams—led by pharmacists—to deliver proactive, patient-centered care. Developed to standardize the use of validated assessment tools like the MPN-SAF TSS and DIPSS-Plus, the PQI enables early symptom identification, risk stratification, and improved workflow efficiency through telehealth-based outreach. Featuring a successful implementation at Cancer Specialists of North Florida, the paper underscores how structured communication, consistent documentation, and patient education enhance care coordination and clinical decision-making for individuals living with myelofibrosis.
